On the 12th of ذو الحجة, الحجاج stone all three الجمرات after Dhuhr (same as the 11th). Those who wish to leave early (ta'ajjul) may depart منى before sunset without needing to stone on the 13th. Before leaving مكة, every الحاج must perform الطواف al-Wida (farewell الطواف).
Pray Fajr in منى and prepare 21 pebbles for stoning the three الجمرات. Continue reciting the takbirat of التشريق after every واجب الصلاة. Make the intention for whether you will depart today (ta'ajjul) or stay for the optional 13th day of stoning. If you intend to leave today, ensure your belongings are packed so you can depart منى promptly after stoning, before sunset.
After the sun passes its zenith (Dhuhr time), proceed to the الجمرات complex. Stone all three pillars in the same order as yesterday: (1) الجمرات al-Sughra (small) — 7 pebbles, then stand and make دعاء facing القبلةh; (2) الجمرات al-Wusta (medium) — 7 pebbles, then stand and make دعاء facing القبلةh; (3) الجمرات al-Aqabah (large) — 7 pebbles, then depart without stopping for دعاء. Say 'Bismillah, Allahu Akbar' with each pebble thrown.
These are the most frequent errors pilgrims make during this stage of Hajj.

After stoning, you have a choice sanctioned by القرآن (Al-Baqarah 2:203). Option A (Ta'ajjul / Hastening): Leave منى before sunset on the 12th. You will NOT need to stone on the 13th. Head directly to مكة for الطواف al-Wida. Option B (Staying): Remain in منى for the night of the 13th and stone all three الجمرات one final time the next day. Staying is considered more virtuous by many scholars, as النبي (صلى الله عليه وسلم) stayed for the 13th.
Before leaving مكة, every الحاج must perform الطواف al-Wida (the farewell الطواف). This is 7 circuits around the الكعبة, performed in the same manner as any الطواف — beginning from the الحجر الأسود corner, keeping the الكعبة on your left. There is no الرَّمَل (jogging) or الاضطباع (baring the right shoulder) in الطواف al-Wida. There is no السعي after this الطواف. After completing the circuits, pray 2 ركعة behind مقام إبراهيم if possible, drink زمزم water, and make your final الأدعية.
After completing الطواف al-Wida and praying behind مقام إبراهيم, take a final look at the الكعبة and make your most heartfelt الأدعية. Ask Allah to accept your الحج, forgive your sins, and grant you the opportunity to return. Depart the Masjid walking backward or looking back at the الكعبة as long as you can see it, as was the practice of some of the Salaf. Leave مكة with a heart full of gratitude, resolved to maintain the spiritual transformation of الحج in your daily life.
While not a rite of الحج, visiting النبي's المسجد (Al-Masjid al-Nabawi) in المدينة is highly مستحب. Many الحجاج visit before or after الحج. Pray in النبي's المسجد, visit the Rawdah, send salutations upon النبي (صلى الله عليه وسلم) at his grave, and visit the graveyard of Baqi'. The visit to المدينة is a separate act of worship and has no connection to the validity of الحج.
