## Diabetes Management During Hajj
Diabetes is one of the most common chronic conditions among Hajj pilgrims. The combination of increased physical activity, irregular meal times, extreme heat, and emotional stress can cause dangerous blood sugar fluctuations. Before Hajj, work with your endocrinologist to adjust your insulin or oral medication regimen for the expected activity level. Pack your glucose meter, test strips, insulin (in a cool bag — insulin degrades in extreme heat), oral medications, fast-acting glucose tablets or gel, and a medical ID card in Arabic and English. During Hajj, monitor blood sugar more frequently than usual — every 3-4 hours and before any major physical activity. Carry fast-acting carbohydrates at all times. Wear well-fitting footwear and inspect your feet twice daily.
## Heart Disease and Hypertension
Pilgrims with cardiovascular conditions face increased risk from the combination of heat, physical exertion, dehydration, and emotional intensity. Carry a recent ECG printout and a letter from your doctor describing your condition, medications, and emergency protocols. Use wheelchair assistance for Tawaf and Sa'i without guilt; preserving your life is a religious obligation that overrides the preference for walking.

The dense crowds, dust, and extreme heat of Hajj can trigger respiratory crises. Carry your rescue inhaler within arm's reach at all times — not in a bag you might be separated from. Pack double the usual supply of all respiratory medications. Saudi Arabia deploys an enormous medical infrastructure during Hajj: over 25 hospitals, 150+ health centers, and thousands of ambulances and medical personnel are stationed throughout the holy sites. Do not hesitate to use these services. Seek immediate help for: blood sugar below 70 or above 300 mg/dL that does not respond to your corrective measures, chest pain or severe shortness of breath, signs of heat stroke (confusion, cessation of sweating, high body temperature), severe allergic reactions, uncontrolled bleeding, any loss of consciousness, or severe diarrhea or vomiting that prevents medication absorption. Keep your medical information and emergency contacts on a card worn on your person, not just stored in your phone.
## The Islamic Perspective on Health Accommodations
Islam explicitly values the preservation of life above the performance of optional ritual elements. Scholars of all four madhahib agree that pilgrims with health conditions may use wheelchairs, perform certain rituals by proxy (tawkeel), delay rituals within permissible windows, and take other accommodations without diminishing the validity or reward of their Hajj.
